Okuma OSP Alarm A 8541 - Spindle phase synchronization interlock
A spindle phase synchronization command was issued while the execution conditions for phase sync were not satisfied. The interlock checks the state of both the spindle being brought into phase and the spindle commanding the sync, blocking the command if either is rotating, indexing, in C-axis mode, in gear neutral, chuck open, or in another conflicting mode.

A spindle phase synchronization command was issued while the execution conditions for phase sync were not satisfied. The interlock checks the state of both the spindle being brought into phase and the spindle commanding the sync, blocking the command if either is rotating, indexing, in C-axis mode, in gear neutral, chuck open, or in another conflicting mode.
Common Causes
Phase sync commanded while one of the spindles was still rotating, indexing, or executing an S command
One spindle in C-axis mode, flat-turning mode, synchronous mode, or lowest speed mode when sync was commanded
Spindle gear in neutral on either spindle
Chuck open on the spindle commanding the phase sync (Code 25)
MacTurn-W/H2 turret: subspindle not in the lower position (Code 30)
Troubleshooting Steps
1Read the code number shown with the alarm: code 11 = Target spindle is rotating; code 12 = Target spindle is indexing; code 13 = Target spindle is in C-axis mode; code 14 = Target spindle is executing an S command; code 16 = Target spindle gear is in neutral; code 17 = Target spindle is in flat-turning mode; code 18 = Target spindle is in synchronous mode; code 19 = Target spindle is in lowest speed mode
2Read the alarm code: codes 11-1A describe a fault state on the spindle being phased (target side); codes 21-2A describe the same states on the spindle issuing the phase sync; code 30 is the MacTurn-W subspindle position condition.
3Codes 11/21 (rotating) or 14/24 (S command active): command spindle stop (M05) on the affected spindle and confirm zero speed before issuing the phase sync command.
4Codes 12/22 (indexing) or 13/23 (C-axis mode): cancel spindle indexing and C-axis mode before commanding phase synchronization.
5Codes 16/26 (gear neutral): engage a valid spindle gear range; phase sync is blocked while the gear is in neutral.
6Code 25: close the chuck on the commanding spindle and verify the chuck close confirmation before issuing phase sync.
7Codes 17/27, 18/28, 19/29, 1A/2A: cancel flat-turning, synchronous, lowest-speed, or the existing phase-sync mode on the indicated spindle, then reissue the command. Code 30: move the subspindle to its lower position first.
